Our Walnut Avenue office is new. The people in it are not. For over thirty years this same team has looked after children across the San Gabriel Valley, and many of the parents we see today were our patients when they were small. We reserve same-day appointments every day, so a child who wakes up unwell can be seen that day rather than next week. Call or text the Chino office early and we will do our best to fit them in.

Dr. Jacqueline Chan and Dr. Eva Chan lead a team of physicians, nurse practitioners and physician assistants who have spent three decades caring for San Gabriel Valley children. The principle has never moved: rigorous medicine, delivered by people children actually like seeing. Our Chino office on Walnut Avenue is the newest chapter of that, now serving Chino and San Bernardino County alongside our West Covina and Diamond Bar locations.

Can I get my child seen today?
Often, yes. We hold same-day appointments each day for sick visits, so call or text us early and we'll do our best to get your child in. We don't take walk-ins, every visit is scheduled in advance, but a quick call or text is usually all it takes.

I have a school form (ie. medicines, allergies, sports physical, annual physical) to be completed. How can I get that done?
Drop off the form to one of our staff members during normal office hours and we will have it completed it for you WITHIN 3-5 business day. Most forms will require a form fee of $35 for our administrative time. If you need it rushed, there is a rush fee of $10.
You can either pick it up in person or the completed form can be e-mailed to you.

Yes, we are accepting new patients. Please complete the new patient registration form HERE and one of our staff will check insurance eligibility before contacting you back to schedule an appointment.
